I acknowledge that I do not have a fever of over 100F, am not currently experiencing any COVID-19 symptoms, have not been in contact with anyone who has tested positive for COVID in the past 2 weeks, and have not tested positive for COVID myself within the last 2 weeks.

I will maintain a distance of at least 6 feet from others at all times.

I will wear a mask at all times.

I am participating as a field staffer voluntarily and without coercion.

I assume all of the risks of field staffing for the DeKalb County Democratic Committee and its directors, officers, members, employees, agents, representatives, and successors (collectively, the “Released Parties”) shall not be liable for any injury or damage to person or property that may arise for me during the course of performing such volunteer activities. On behalf of me and my assigns, heirs, legal representatives, executors, agents, and any organization or entity with which I am affiliated, I hereby RELEASE AND DISCHARGE THE DEKALB COUNTY DEMOCRATIC COMMITTEE AND THE RELEASED PARTIES FROM ANY AND ALL CLAIMS, LIABILITIES, RIGHTS, DEBTS, SUITS, COMPLAINTS, PROMISES, CROSS-CLAIMS, CONTROVERSIES, DAMAGES, AND/OR DISPUTES asserted or unasserted, known or unknown, that arise in any way from my involvement with the DeKalb County Democratic Committee.

This Agreement shall be governed by the laws of Georgia for all purposes, without regard to its conflict of law principles, and shall fully bind Volunteer’s heirs, assigns, executors, and legal representatives. Any dispute arising from this Agreement shall be brought only in the state or federal courts located in Georgia, and the Parties agree to the jurisdiction and venue of those courts for any such dispute. By signing up to be a field staffer with the DeKalb County Democratic Committee, I represent and warrant that (a) I have the power and authority to execute and deliver this Agreement, (b) I am not being compensated by any third party to provide services to DCDC; and (c) I have read the foregoing.
